Home Care Services

 

As a Registered Nurse, I am a licensed medical professional with a diverse range of skills including: evaluating patient conditions, care coordination and managing complex patient cases. I also offer Virtual Health Consultations on Zoom and Telehealth Consultations via telephone if client prefers. Virtual/Telehealth Services are offered to anyone in the lower mainland and packages are available for ongoing services.

 

 

Skilled Nursing Services and Support

With my oversight I will perform in-home health assessments and a wide range of home care services that meet either short-term or ongoing medical needs. Services may include, but are not limited to:

• Acute or chronic disease management

• Holistic Health checks and assessments

• Medication management, administration and oversight

• Resource Referrals and Management- (Physio, OT, Speech Therapy, GP, Dietitian, Social Work, Podiatrist etc.)

• Monitor and assess vital signs

• Wound Assessment/Care

• Alzheimer’s/Dementia Management and Monitoring

• Blood sugar testing and monitoring

• Post- Surgery Care

• Care Planning and Case Management

• Personal Care (Lending a hand with bathing, grooming and toileting assistance)

• Lifestyle support (Housekeeping, meal prep, laundry, grocery shopping, errands and companionship)

• Management/Transportation to Health Appointments
